RE: Ravenscroft Medical Centre, 166 Golders Green Rd, Golders Green, London NW11 8BB  

I am writing to you because you are a registered patient at Ravenscroft Medical Centre. I would like to inform you about a potential change to the location of the practice and to give you the opportunity to share your views on this change.

The practice is proposing a relocation to Olympia House, Armitage Road, Golders Green London NW11 8RQ. This is 0.2 miles from the current practice site.

We will review your feedback and listen to the views of other stakeholders such as Healthwatch, local councillors and MPs. We will also write to all practice patients again to update them with further information and the outcome of this survey. 

Please note that the deadline to complete the survey has been extended from the 16 February to Sunday 28 February 2021.
