Thank-you for completing this situation report for your practice. Please focus on the situation for your practice for this week and respond by the end of Tuesday, 24th November 20 . Your answers will be reviewed by your borough team on Wednesday. If any issues you are raising require a response today please also contact your borough team directly. 

We anticipate this survey will take no more than five minutes to complete. In future weeks, if there has been no change since the previous week you will have the option to indicate that there has been no change and submit the survey without answering any further questions.
