Thank you for your interest in joining the Service User and Carer Working Group. This application form is just a chance to find out a bit more about you, and why you are interested in joining the group. 

It will only take about 15 minutes, and most of the questions are just ticking boxes. If you have any problems, please do get in touch. You can also save the form and finish it later if you would prefer.
